For remote-only work, indicate the address of the responsible corporate office/facility.

About Alternative Phone/Email for Contacts

Section titled “About Alternative Phone/Email for Contacts”

Upon receipt of payment, IAQG Authentication will email all provided contacts to verify your audit experience. If IAQG is unable to reach the contact at the primary email address provided, you will have the option to switch to the alternative email. IAQG may use the provided primary and alternative phone numbers to further verify claimed audit experience, as necessary.

If I only participated in one Surveillance audit for a given facility within a cycle, can I count it as audit experience?

Section titled “If I only participated in one Surveillance audit for a given facility within a cycle, can I count it as audit experience?”

No, the IAQG only permits surveillance audits to be counted if completing both halves of the surveillance cycle, collectively auditing the entire QMS. If entering Surveillance audits, provide matching “Surveillance 1” and “Surveillance 2” audits.

Will first-party (internal) audits be counted towards experience requirements?

Section titled “Will first-party (internal) audits be counted towards experience requirements?”

No, first-party audits will not count towards minimal experience requirements. However, applicants may optionally provide internal audits as additional audit experience evidence, beyond the required minimum, to support their application.

The supervisor and/or lead auditor no longer work at the company and can no longer verify my experience. Can I still claim the audit experience?

Section titled “The supervisor and/or lead auditor no longer work at the company and can no longer verify my experience. Can I still claim the audit experience?”

IAQG Authentication requires an outside authority to verify your audit experience. However, these parties do not necessarily need to still be employed by the company in question. Applicants may provide a contact’s personal email address, or one for the contact at their new employer, provided the contact is still willing to verify the audit experience. If the assigned Lead Auditor is no longer available to verify the audit experience, applicants may list a responsible supervisor and explain the situation in the comments. Ensure any contact information has been provided with the consent of the individual to be contacted by IAQG Authentication for this purpose.
